● All student medications whether prescription or over-the-counter, must have a new “Physician’s Authorization for Student Medication” form for each school year which the parent also signs. Your child’s doctor will have these forms in their office.
● Students may not carry any medication on their person or their belongings without a Physician’s Authorization form on file with the clinic that designates carry-on status. Carry-on status is limited to EpiPens, inhaler and diabetic medications/supplies.
● All medications must be stored in their original container/box. This applies to all EpiPens.
● Prescription medication must have the pharmacy label attached.
● All medications must be transported to school by an adult.
● Nurses are prohibited from administering expired medications.
● All field trip medications must be submitted to the nurse at least 2 days prior to the field trip.
